Hi all, I would like to introduce myself as the Central Coast Local Health District Intellectual Disability Care Coordinator (IDCC). Hi all,

I would like to introduce myself as the Central Coast Local Health District Intellectual Disability Care Coordinator (IDCC). This newly created position is to provide specialised consultation and education to CCLHD health clinicians on the assessment and management of a person with intellectual disability.

Also to provide consultation and education to external stakeholders GPs, Department of Education, Juvenile Justice, Department of Communities and Justice and local Central Coast Non-government Organisations. IDCC will provide capacity building to enhance knowledge and skills in working with people with an intellectual disability. For example, this can be working with GPs one on one in relation to a particular patient, or providing skills training or other education sessions to groups.

As the IDCC, my aim to address the gap in local health care for people with an intellectual disability of all ages and create pathways to increase the systemic capacity of the region to better serve consumers, carers and their families.

Also to devise and implement systems to encourage collaboration and information sharing across clinicians, specialties, service providers and agencies.

IDCC is also the conduit between CCLHD and Hunter New England (HNE) Specialised Intellectual Disability Health Team (SIDHT).

Furthermore, I work as a conduit between CCLHD and HNE Specialised Intellectual Disability Health Team (SIDHT) to lead the roll out of the Specialised Intellectual Disability Health Team outreach assessment clinic for the district. The SIDHT will include multidisciplinary assessment and care planning to manage complex consumers with an intellectual disability and a current unresolved health issue that cannot be addressed through usual care.

I have attached SIDHT Factsheets for your perusal/ distribution as you see fit.

Please do not hesitate to contact me if you require any further information or to make referrals to myself and SIDHT.
